Your General Membership Meetings Can Provide Valuable
Recertification Credits For SR/WA designation or any
Certification Program.
The form consists of two parts:

A. Pages 1&2 – Confirmation of Attendance
1. One of the Chapter Officers must certify and put the
number of hours the events was approved for,
2. The form must be signed dated by one of the Chapter
Officers,
3. Must be provided to the attendees of the event

B. Pages 3 & 4 – Sign-in Sheet
1. May be used to record attendance at the event,
2. Must be retained at the Chapter/Region level.
